Partners in Change Hub

The Partners in Change Hub brings together the specialist support to enhance social work practice in one hub, which will work across the social work pods. This means that the Lead Practitioners, Clinical Psychologists, Chance 2 Change workers, Therapists, Partners in Change workers for adult substance misuse and mental health, EPAP and Looking Forward will be working together to make sure that you and the families you support get the best service from them as easily, and as early, as possible.

The purpose of the hub is to provide specialist support to social workers and families in order to help families to change.  The hub will offer help at any stage of our processes and will be focused on complex situations.  The hub will strive to make sure that social workers and families receive the right support without the need to make multiple or duplicated requests for help.  The support offered by the hub will be consistent with the principles of our model of practice and will be focused on delivering specialist support via the social worker’s relationship with the family.  Practitioners from the hub will work in a way that develops the skills and knowledge of social workers at the same time as improving outcomes for families.
